The Registration Forms to Attend Lions Camp Merrick are available here for download. The forms are in PDF format. You will need the Adobe Acrobat Reader to read and print the registration form. The Acrobat Reader is available at no charge to you. If you do not already have the Acrobat Reader, click here to go to the Adobe web site and download the reader and install it. Then return here, click on the form below to download it to your computer. You may then read, print, fill it out and then either mail or fax it to the camp. Mailing addresses and fax numbers are included on the forms.
